Penelitian bertujuan untuk mengetahui pelaksanaan program Imunisasi di Puskesmas Kuamang tahun 2016. Metode penelitian yang digunakan metode kualitatif dengan desain studi evaluatif. Penelitian dilakukan bulan Agustus 2016 di Puskesmas Kuamang, pengambilan sampel yang digunakan bersifat purposive sampling. Pengumpulan data dilakukan dengan wawancara mendalam terhadap kepala puskesmas, pengelola program, masyarakat dan telaah dokumen. Hasil penelitian ini dari komponen input, Kebijakan atau regulasi dari dinas kesehatan Kabupaten pasaman untuk mendukung pelaksanaan program Imunisasi sudah ada bahkan dari Mentri Kesehatan RI No. 42 tahun 2013 tentang penyelenggaraan Imunisasi. Keaktifan petugas dan kader masih kurang. Sarana prasarana Imunisasi tidak ada tempat khusus. Pada komponen process, secara umum kegiatan Imunisasi di Puskesmas Kuamang tidak terlaksana secara maksimal karena kendala keaktifan pengelola program maupun petugas kesehatan yang tidak konsisten dengan rencana kerja sehingga ada kegiatan yang tidak terlaksana, monitoring dan evaluasi belum berjalan optimal serta masih kurangnya kesadaran dan partisipasi masyarakat untuk berpartisipasi. Pada komponen output, pelaksanaan program Imunisasi belum terlaksana dengan baik dan belum mampu meningkatkan cakupan pelayanan kesehatan setiap tahunnya. Kesimpulan dari penelitian di atas masih adanya kendala dalam pelaksanaan Imunisasi di Puskesmas Kuamang pada komponen input dan process dari pelaksanaan kegiatan pelaksanaan Imunisasi. Perlu adanya dukungan dari manajemen puskesmas dalam rangka meningkatkan kualitas dan kuantitas tenaga pelaksana Imunisasi. Menurut Data Riset Kesehatan Dasar (Kementrian Kesehatan RI, 2018) dimana sebanyak 11,7% remaja di Indonesia mengalami menstruasi tidak teratur. Salah satu faktor yang menyebabkan siklus menstruasi yang tidak teratur yaitu status gizi, aktifitas fisik dan stress. Beberapa faktor yang menyebabkan siklus menstruasi tidak teratur yaitu Status Gizi (diet), Stress (kecemasan) dan aktivitas fisik (Han and Goleman, Daniel; Boyatzis, Richard; Mckee, 2019). Tujuan penelitian ini untuk mengetahui faktor-faktor yang berhubungan dengan gangguan siklus menstruasi pada remaja putri. Penelitian dengan desain cross sectional ini dilakukan pada populasi MTS dan MAN. Penentuan sampel dilakukan dengan propotional stratified random sampling dan didapatkan besar sampel sebesar 34 remaja putri. Data terkait siklus menstruasi didapatkan dengan wawancara menggunakan kuesioner. jenis penelitian survey analitik. Analisis univariate mengunakan analisis deskriptif dan bivariat mengunakan uji chi square. Hasil penelitian menunjukkan bahwa responden siklus mentruasi tidak teratur yang memiliki sebagian besar status gizi normal (64,7%), responden aktivitas fisik berat sebagian besar (88,2 %), Responden dengan cemas ringan sebagian besar (38,2 %). Berdasarkan hasil uji statistik, diketahui bahwa terdapat tidak ada hubungan antara siklus menstruasi dengan status gizi p value (0,719), aktivitas fisik p value (0,269), OR (1,000) dan stress p value (0,377). Terdapat tidak ada hubungan antara siklus menstruasi dengan status gizi, aktivitas fisik dan stress. Saran untuk remaja putri pada aktifitas fisik bisa mensiasati waktu istirahat minimal 8 jam sehari dan pada stress pihak pesantren bisa memberi aspresiasi remaja putri dengan acara kuliner, jogging, berenang ke tempat umum dan acara organisasi bisa dilakukan di alam terbuka.
